Job SummaryPlans, coordinates and implements the care of the pre and post-VAD patient, in both the inpatient and outpatient setting. This includes but is not limited to patient education, staff education, surgical coordination, clinic visits, and recordkeeping.

Responsibilities:
- In conjunction with heart failure physician/ APRN, assesses patient in an inpatient setting or outpatient clinic.
- Facilitates plan of care and symptom management with inpatient /outpatient staff.
- Reviews medical information from referring physician and initiates appropriate communication.
- Prior to procedure, coordinates with patient's heart failure physician/ APRN and referring physician for evaluation of patient for advanced therapy. .
- Assembles test results of evaluation and arranges further testing, if indicated.
- Plans, conducts and evaluates educational activities regarding ventricular assist devices for potential recipients, families and professionals.
- Triages patients during the entire VAD process for any medical and psychosocial issues. Is available by pager and by phone in order to provide immediate assessment and delivery of care in a timely and cost-effective manner.
- •Coordinates information flow among medical, ancillary, nursing, financial and insurance offices.
- Collaborates with physicians to provide direct care to patients and plans and evaluates the delivery of care.
- •Manages the health care of patients with VADs.
- Assists in obtaining medications and supplies from the appropriate source according to the patient's insurance coverage.
- Manages equipment, supplies, and charges in conjunction with hospital liaisons.
- •Assists in reinforcement of discharge information, and trains community for patient discharge.
- Arranges follow up at CCF
- Supports requirements for continued VAD Certification by Joint Commission.
- Participates in research by the implementation of protocols and the collection of data.
- •Facilitates procedures which enable the implementation of studies with various interdepartmental components.
- Provides education to patients ,caregivers (internal and external) with specific focus on device management and plan of care following implantation of MCS/LVAD. Educates on post care responsibilities and how to respond to emergency situations to provide optimal outcomes following implantation.
- Accompanies MCS/LVAD patients to procedures and surgeries.
- Other duties as assigned.
Education:
- Completion of Registered Nurse program and must obtain Bachelor’ Degree in Nursing (BSN) within three years of hire date as per Cleveland Clinic policy.
Languages:
- English
Certifications:
- Current state licensure as a Registered Nurse (RN).
- Basic Life Support (BLS) Certification
- Advanced Life Support (ACLS) Certification

Work Experience:
- Two years of experience in acute care, medical/surgical, or ambulatory setting within a hospital environment with 1year experience caring for MCS/LVAD patients.
Physical Requirements:
- Requires full range of body motion, manual finger dexterity and coordination.
- Requires working under stressful conditions and working irregular hours.
- May be asked to assist with call on a rotating basis.
Personal Protective Equipment:
- Follows Standard Precautions using personal protective equipment as required for procedures.
