Major Responsibilities:
Serves the department as a Medical Social Worker. See Medical Social Worker Job Description.
1)Maintains professionalism required by Leadership staff while functioning in Frontline Associate Role.
Assist in leadership and support for day to day operations of the Care Management/Social Work departments.
1)Reviews and manages daily schedules in collaboration with Manager to meet balanced schedule based on target ratios. Reviews and approves requests for PTO within established guidelines. Reviews weekly timekeeping, approves/updates payroll as required.
2)Serves as an expert user as appropriate for epic, Midas, ECIN, and Milliman as it pertains to Social Services.
3)Functions as staff preceptor and resource for assessment and interventions related to Medical Social Work practice. Provide feedback, coaching, and mentorship to Social Workers and Discharge Counselors. Performs staff audits and provides prompt constructive feedback. Provide ongoing monitoring of work performance and develop learning plans for associates not meeting standards of performance in collaboration with Manager of Care Management/Social Services.
4)Provide input into the development and implementation of policies and objectives related to the department.
5)Performs other duties as assigned.
Demonstrates job knowledge: Assess patients for psychosocial, financial, and discharge planning needs along the continuum of care.
8)Utilizes Care Management software systems to collect and analyze data designated by management.
9)Manages/oversees patients with length of stay’s >20 days and collaboratively assist Care Management team with discharge planning.
1)Ability to apply best practice approach to Social Work assessments and discharge planning.
2)Identify and remove barriers to discharge early in patient's stay and formulate plan of care in conjunction with the patient, family, and other members of the healthcare team, payers, and community resources.
3)Work collaboratively with the Care Management team on complex patients to make sure all identifiable needs are addressed before discharge.
4)Facilitates the management of the patient's fiscal resources, in collaboration with the interdisciplinary team.
5)Attends multidisciplinary and physician rounds and daily team communications.
6)Initiates referrals to other members of the healthcare team regarding psychosocial, financial, and discharge planning needs.
7)Serves as a resource for the patient and healthcare team regarding discharge planning, financial, and community resources.
Demonstrates advanced level of knowledge in Medical Social Work.
1)Maintains clinical/business knowledge through continuous learning.
2)Demonstrates an understanding of Medicare/Payer requirements as it relates to regulatory requirements and policies and procedures that affect patient movement through the continuum of care.
3)Provides input into annual performance appraisals, staff comptency, assignments, and process improvement. Participates in recognition and rounding, coaching/mentoring, and progressive discipline of Associates in conjunction with the Manager.
4)Participates in activities related to recruitment, interviewing, hiring, retention, and professional development of qualified staff.
Evaluation of effectiveness of Social Work strategies.
1)Monitor the effects of educating the interdisciplinary team members on discharge planning requirements and quality transitions of care.
2)Routinely shares data with staff as it relates to the business side of healthcare, patient movement, and how it effects hospital reimbursement.
Licensure:
Social Worker, Clinical
Education/Experience Required:
• MSW • 1-2 years experience in social work • Experience in progressive leadership role Project implementation and or educational leadership experience
Knowledge, Skills & Abilities Required:
• Counseling and psychotherapy skills. • Knowledge of post-hospital care resources.
• LSW-IL
Physical Requirements and Working Conditions:
Occasionally requires carrying weights of 8lbs (laptop) -Resiliency and flexibility to work within rapidly changing environment. -Urgent/emergency response to regulatory requests -Expertise in relationship building -Excellent organizational skills -Ability to prioritize independently.

Pay Range
$32.45 - $48.70
