Humana is seeking a Durable Medical Equipment (DME) Coordinator to join the Inclusa team to serve the Wisconsin Family Care (FC) program. This role is a part of the DME team comprised of coordinators and assistants. The DME focus area within Care Management Support is responsible for assisting teams through critical thinking and providing recommendations for the most effective and cost-effective options when purchasing high dollar items- complex Durable Medical Equipment.

  • Act as liaison between stakeholders to provide a recommendation to the interdisciplinary team (IDT) on the most effective and cost-effective ways to meet member needs. Requires excellent communications skills with the ability to multi-task and provide follow-up as needed.
  • Provide guidance, expertise, and experiences related to complex durable medical equipment (DME).
  • Build and maintain resources and tools to support objectives.
  • Provide consultation and support to implement existing organizational policies and procedures.
  • Research and evaluate all applicable options for durable medical equipment, to guide the referral process from start to finish.
  • Track referral progress and development within established timeline expectations.
  • Maintain knowledge of latest trends and available resources maximizing alternate funding sources.
  • Complete applicable documentation.

Required Qualifications

  • Three (3) years' experience working with Family Care target population within the last ten years.
  • Previous experience with complex Durable Medical Equipment
  • Proficient with Microsoft Office Applications
  • Must reside in the State of WI

***The Family Care target group population is defined as: frail elders and adults with intellectual, developmental, or physical disabilities***

Additional Information

  • Workstyle: Home. Home workstyle is defined as remote but will use Humana office space on an as needed basis for collaboration and other face-to-face needs.
  • Work Location: must reside in Wisconsin.
  • Typical Work Days/Hours: Monday – Friday, 8:00 am – 4:30 pm CST

Limited Geography Remote - This is a remote position but located within a specific geography.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
