SUMMARY:

The Clinical Community Educator will be a Licensed Professional Nurse that has the overall primary responsibility of promoting the (Agency) Hospice brand through the development of clinical relationships with acute care partners, post-acute care partners, senior living communities and other referral sources.  The Clinical Community Educator assists in supporting the transition of identified patients in need by supporting discussions with their families and completing the initial election statement confirming consent for care. 

QUALIFICATIONS & ATTRIBUTES:

  • RN or LPN with a minimum of 2 years of experience in sales and marketing in the hospice continuum of care (experience supporting and developing GIP programs desired).

  • Preferred Registered Nurse (RN) Licensure in the state of services.

  • Ability to create positive impressions and communicate with a variety of people. Maintain effective communication with patients, families, physicians and co-workers. Compassionate and caring while working with patients.

  • Ability to make appropriate clinical judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.

  • Verbalizes knowledge of hospice and palliative care Nursing, State and Federal regulations.

  • Differentiates Hospice from acute care nursing as noted by verbal report during IDT meeting, and implementation of interventions supporting the hospice plan of care. 

  • Utilizes the Nursing Process in a holistic approach to identify complex needs and to develop and implement creative strategies for meeting those needs while recognizing, reporting, and documenting signs and symptoms of decline.

  • Documents data relative to complex or sensitive psychosocial, medical-legal, physiological states.

  • Develops, implements, and evaluates teaching plan. Incorporates teaching plan into daily care and documents as well as providing educational support to referral sources as needed. 

  • Performs care adhering to the professional standards of practice and hospice standards of care.

  • Assists in developing or revising policies and procedures as requested. 

  • Participates in audit process as requested to ensure compliance with State and Federal regulations.

  • Assists in compliance committees and meetings as requested.

  • Disciplined style of work ethic. Ability to prioritize and be timely. Works efficiently.

  • Ability to follow directions and work as a team member.

MAJOR AREAS OF RESPONSIBILITY:

  • Provides support to ensure that all eligible patients receive care, barriers to admissions are addressed and agencies growth and profitability is maximized. 

  • Attend pertinent meetings and visit key referral sources with Community Relations Representatives, developing the referral relationships of the agency within the community, in accordance with (Agency) Hospice policies and procedures.

  • Reviews the patient’s medical record and gathers clinical information from the current care team and patient.  

  • Conducts an ‘at the bedside’ meeting with the patient and caregiver to identify and discuss the patient’s reason(s) hospice, qualifications and potential admission plan.

  • Encourages patient and/or caregiver engagement in his/her own care by providing education regarding the patient’s medical condition, disease and symptom management, referral source and community resource options.

  • The role includes explaining benefits, gathering information towards the determination of eligibility, completing the hospice care elections statement and beginning the admission process as appropriate.

  • Demonstrates initiative in applying sound principles of management in planning, organizing, and delivering care.

  • Collaborates and communicates with all members of the interdisciplinary team.

  • Participates actively in meeting the goals and objectives of the hospice organization.

  • Performs and documents care adhering to the professional, hospice, regulatory body, and nursing standards.

  • Demonstrates an ongoing commitment to her/his own professional growth.

  • The Clinical Liaison partners with the Community Relations team to develop and maintain referral source relationships and to assist with timely interventions if issues arise.

  • Maintains active RN or LPN license in the state of services.

  • Other duties as assigned.

COMMUNICATION AND INTERPERSONAL SKILLS:

  • Maintains a friendly yet professional demeanor when interacting with hospital and facility staff, the patient, caregiver, and family, as well as, with members of the interdisciplinary team and office support staff.

  • Respects the professional boundaries of communicating personal information with patient and caregiver. 

  • Utilizes effective communication skills both verbal and nonverbal with the patient/family. 

  • Serves as patient/family advocate. 

  • Initiates and documents follow-up to patient/family needs. 

  • Pursues job with enthusiasm and positive approach. 

  • Utilizes courteous and standardized approaches to phone communication. 

  • Maintains confidentiality of privileged information and supports the patient’s rights to privacy. 

  • Acts as a resource person to other staff members as needed. 

  • Utilizes appropriate supervisory and resource persons for problem-solving and identifying specific team needs. 

  • Submits to supervisors and agency leadership a written weekly report capturing identified metrics for data entry in playmaker by the Community Relations Director and in preparation for monthly review and discussion. 

  • Meets with supervisors and agency leadership each month to review and discuss identified metrics.
