Care Coordinator

About the Role: 

The Unite Us Care Coordinator plays a pivotal role in supporting customers who have purchased social care coordination services by leveraging the Unite Us Platform to ensure the delivery of high-quality care coordination services. This position entails interacting directly with clients seeking services and collaborating with partner organizations to facilitate resource allocation and support for clients. Care Coordinators are empathic individuals who serve as platform experts, proficient in conducting client needs assessments, guiding clients in accessing social determinants of health resources, and closing gaps in care.

Care Coordinators are Unite Us Platform experts and have the ability to conduct client needs assessments and assist clients in identifying social determinants of health resources to meet their needs.

What You'll Do:

• Manage a caseload of customers' clients that averages 50 referrals completed weekly. This will comprise initial calls, assessments, follow-up, and closing the loop with partner organizations.  
• Utilize the Unite Us Platform to facilitate seamless care coordination services for customers, ensuring contractual requirements are met
• Conduct intake and needs assessment screenings for clients seeking assistance to understand their needs and preferences for care coordination services.
• Collaborate with partner organizations to identify and connect clients with the appropriate services based on their needs, eligibility, and geographic preference
• Serve as a liaison between clients and service providers to ensure effective communication and coordination of care
• Conduct follow-up with clients and service providers to ensure receipt of services/benefits 
• Under the social services landscape and provide supportive feedback to community-based-organizations serving Care Coordination clients to ensure network standards are met
• Continuously expand knowledge of community resources, services, and programs to understand community issues and build relationships with other users of the Unite Us platform
• Be a champion of customer-centric culture and deliver excellence in customer experience by meeting and exceeding service expectations and solutioning workflows that drive impact 
• May support special projects and backend workflows that help drive success for customers, communities, and consumers 

You’re a great fit for the role if:

• Minimum of one year of experience in navigating social services as an individual, case manager, social worker, or care navigator worked at community based org, knows the landscape
• A degree in social work or a relevant field preferred
• Clinical credentials preferred Case mgmt/Care Coord cert/CHW
• Empathy and passion for helping vulnerable populations
• Quick learner who is excited to become an expert in new technologies
• Flexible, meeting the needs of the dept. Shift priorities/balance/bounce
• Ability to set priorities and work independently while following standard operations procedures
• Friendly and compassionate; comfortable communicating directly with clients in need and community-based organizations
• Resourceful and fast-paced; able to conduct quick, exhaustive online research

Environmental Job Requirements and Working Conditions:

• This position is remote, U.S. based. Central Time Zone or Mountain Time Zone is strongly preferred.
• The target pay range for this role is: $22-$24 hourly (approximately $46k-50k annually). This salary range represents our target hiring range for this role. The proposed salary will be dependent on the candidate's skills, experience, and competencies, as well as location.
