Their novel whole-person care model - social, clinical, behavioral - is quarterbacked by a multidisciplinary care team across their network of shelters, food pantries, and other community partners leveraging Arc, their proprietary care management platform. Pair Team is one of the largest providers of a high-needs case management benefit in California and on track to build the country's largest clinically-integrated network of community-based organizations. They have demonstrated an ability to engage a hard to reach patient population and address gaps in care, including a 56% reduction in ED visits and a 26% reduction in hospital admissions.
