A Brief Overview
Coordinates the initial screen and triage functions in collaboration with Masters prepared Social Workers by providing initial screenings for patients and families and the provision of concrete resources and referrals. Collaborates closely with the multidisciplinary staff to provide a coordinated experience to patients and families. Receives a high volume of inquiries from a variety of sources and gathers pertinent medical and psychosocial information to help determine level of acuity and appropriate resources. Plans and implements patient disposition and other social work case management and utilization management activities for patients. Works under the direct supervision of a social worker.
What you will do

  • Supports Social Workers in identifying and screening for high risk populations for psychosocial concerns, repeat admission and barriers to safe discharge.
  • Based on assessed need, provides concrete resources to families across the institution, including cab and meal vouchers.
  • Educate patient and family regarding community based services and referral process.
  • Provide community-based referrals for support and resources.
  • Partners with team and families to ensure understanding of care plan and next steps during admission and after discharge.
  • Communicate assessments with multi-disciplinary team for ongoing follow-up during admission.
  • Initiates other referrals for the social worker, such as: demographics, insurance information and preliminary diagnosis and orders.
  • Collaborates with all members of the health care team, family and managed care organizations and makes appropriate community referrals and follow-up appointments.
  • Supports social workers in coordinating transportation and referral needs of patients and families.
  • Review client charts and document intervention status on charts.
  • Fax referrals for community-based services or other referrals and maintain communication/referral log.
  • Maintain paper forms, office supplies and equipment used by the social workers.
  • Performs other duties as assigned.

Education Qualifications

  • Bachelor's Degree behavioral health, social sciences, education Required

Experience Qualifications

  • At least two (2) years experience in healthcare or community resource agency Preferred

Skills and Abilities

  • Proficient with Microsoft Word and Excel.
  • Knowledge of PowerPoint.
  • Ability to work independently with minimal supervision.
  • Ability to be proactive in following up on projects and tasks.
  • Excellent phone etiquette.
  • Ability to handle confidential materials.
  • Ability to handle multiple tasks.
  • Excellent organizational skills.
  • Knowledge of medical terminology and protocol.

Licenses and Certifications

  • Licensed Social Worker (LSW) (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Preferred
  • Membership in the Academy of Certified Social Workers (ACSW) - Academy of Certified Social Workers (ACSW) - upon hire - Preferred
