$55,000 to $67,500 Per Year
You'll Be Responsible For:
- Overseeing the work of the Social Service Designee
- Helping residents and families adjust and solve problems regarding their social, emotional, and financial needs
- Assisting with Room and Roommate Changes
- Attending Medicare Meetings
- Discharge planning for patients by arranging for follow up community services, home health referrals and alternative placements,
- Advocating for residents and families regarding resident rights issues.
- Conducting in-service training to educate staff about the emotional & social needs of residents, and offer methods for better serving residents and families
- Attending administrative staff meetings, morning reports, Performance Improvement meetings, resident care-related meetings, and quarterly staff meetings
- Maintaining departmental records and documentation as required
- Coordinating and updating resident care conference schedule
- Assisting residents and families with Medicaid approval and reapplication
- Providing Medicaid Pending and Patient Liability related follow-up
You'll Need To Have:
- A Bachelors Degree in Social Work
- Active Licensed Social Worker Certification in the state of Ohio
- Long Term Care Experience or willingness to learn
- Excellent communication and listening skills
- The ability to identify residents’ social, emotional, and financial needs
