We are growing fast and are looking for a Customer Success Manager – Clinical Implementation to drive adoption, engagement, and successful outcomes for our clients in post-acute care settings.

Role Overview

As the Customer Success Manager – Clinical Implementation, you will play a pivotal role in onboarding, training, and optimizing the use of Circadia Health’s platform for SNFs and home health agencies. You’ll collaborate closely with clinical teams, administrators, and technology stakeholders to ensure seamless implementation, drive user engagement, and enhance patient outcomes.

This is a hybrid role based in New York City, with onsite visits to post-acute care facilities as needed.

Key Responsibilities

Clinical Implementation & Training

  • Lead the end-to-end onboarding and implementation of Circadia Health’s platform across Skilled Nursing Facilities and Home Health agencies.
  • Conduct workflow analysis and customization to align our solution with the operational and clinical needs of each facility.
  • Train clinical staff (RNs, LPNs, therapists, and care coordinators) and administrative teams on best practices for leveraging our platform.
  • Provide ongoing support and education to drive adoption and maximize value.

Customer Success & Engagement

  • Serve as the primary point of contact for post-acute care clients, ensuring they achieve measurable clinical and operational improvements.
  • Develop strong relationships with key stakeholders, including DONs (Directors of Nursing), Medical Directors, Administrators, and Home Health Leaders.
  • Monitor customer usage, adoption trends, and clinical outcomes, proactively addressing barriers to engagement.
  • Organize check-ins, QBRs (Quarterly Business Reviews), and clinical outcome reviews to showcase impact and identify areas for improvement.

Process Optimization & Product Feedback

  • Work closely with product and engineering teams to provide frontline feedback on workflow enhancements and feature requests.
  • Identify opportunities for automation and AI-driven insights to improve patient care.
  • Support internal teams with data-driven case studies, ROI analyses, and customer success stories.

What We’re Looking For

Experience in Post-Acute Care (SNFs & Home Health) – Minimum 3+ years in a clinical, customer success, or implementation role within Skilled Nursing, Home Health, or a healthcare technology company.

Clinical & Operational Understanding – Strong grasp of nursing workflows, PDPM, rehospitalization prevention, and post-acute reimbursement models.

Tech-Savvy & Analytical – Comfortable working with AI-powered platforms, EHRs (PointClickCare, MatrixCare, Homecare Homebase), and clinical data dashboards.

Customer-Centric Mindset – Proven ability to drive user adoption, solve customer pain points, and build long-term relationships.

Project Management Skills – Experience with implementing new technologies in healthcare settings and managing multiple stakeholders.

Travel Flexibility – Ability to travel onsite to SNFs & home health agencies in the NYC region as needed.
