Aledade is seeking two Program Managers to support the development and advancement of activities through the Aledade Community Health Center, Center of Excellence. The CHC COE is designed to provide training, education, and program support, both internally and externally, to drive CHC success in value-based care. These Program Manager roles require experience working with Federally Qualified Health Centers, understanding of population health and value-based care principles, and knowledge of collaborative learning models.
The Program Manager is responsible for developing and facilitating educational activities tailored to Community Health Centers (CHCs). This includes, but is not limited to, preparing and delivering webinars, organizing roundtable or collaborative events, and planning for in-person learning events. This may also include working with internal departments to facilitate the success of supporting CHCs, such as Product and Technology, policy, and others. The Program Manager is expected to create and implement change packages to support improvement efforts and utilize data effectively to drive and sustain measurable improvements.
Program managers in the Center of Excellence (COE) will need to be skilled with data management and analysis using tools such as Tableau, Salesforce, and Excel. This role works with both internal and external partners, and requires strong organizational, communication, and analytical skills to ensure the successful execution and achievement of program goals.

We are flexible with respect to geographic location, and the ideal candidate will be comfortable working remotely/work from home within the US, with periodic travel required.
Primary Duties:
- Develop, coordinate, and implement large-scale educational initiatives, including newsletters, webinars, roundtables, and regional collaboratives, for both internal and external audiences.
- Plan and coordinate resources and logistics for in-person CHC events.
- Develop and execute on an awards program for CHC high performers.
- Track performance for CHCs on ACO performance, and collaborate with internal stakeholders at Aledade to ensure alignment and consistency in performance tracking efforts.
- Collaborate with Market Teams to assess and monitor CHC satisfaction with Aledade programs and offerings.
- Other duties as assigned to ensure maximal coordination across Aledade teams.
Minimum Qualifications:
- Bachelor’s degree and/or 7+ years of healthcare work experience
- Experience working with FQHCs
- 2+ years of progressive value-based care experience
- Understanding of improvement science, such as the IHI-Breakthrough series
- Demonstrated experience managing large-scale events
- Able to thrive in a fast-paced, startup environment with occasional ambiguity
- Proven ability to successfully manage multiple programs, coordinating and collaborating across projects and teams.
- Strong and clear writer
Preferred KSA's:
- Ability to support high level data visualization and analytics
- Experience with adult learning principles
Physical Requirements:
- Sitting for prolonged periods of time. Extensive use of computers and keyboard. Occasional walking and lifting may be required.
- Willingness to travel 10-15% of the time, as needed, to support CHC events.
