We are Aspire Living & Learning, a non-profit human services agency making a meaningful difference in the lives of adults and children with autism and other disabilities.
Aspire Living & Learning is currently seeking a Case Coordinator to become a collaborating member of an individual's care team. The Case Coordinator is integral to ensuring excellent services to the people supported by Aspire. The Case Coordinator carries a caseload assigned by the supervisor, director, or department head. Individuals on the caseload may have varying degrees of need or oversight.
Program Oversight:
Monitor, verify, and document various levels of programmatic or clinical support or service provision. This may include collecting, aggregating, or reporting on service-specific data points or data collection procedures. Ensures accurate and timely reporting.
Administrative Tasks:
Complete all required documentation, meeting deadlines. Ensure relevant sections of electronic record is up to date and accurate. Facilitate and/or attend all team meetings.
Communication and Consultation:
Effectively communicate in both verbal and written form with necessary stakeholders. Accept feedback from supervisors to ensure the quality of program implementation. Communicate and collaborate with both internal and external team members and stakeholders. Maintain ethical and professional boundaries and guidelines. Work cooperatively and collaboratively with all necessary stakeholders.
Education, experience and other requirements:
- High school diploma or GED
- Strong Computer Skills
- Valid driver’s license and safe driving record
- Excellent interpersonal, organizational, and communication skills
- Must have satisfactory background checks
- Background in clinical/behavioral supports preferred
