The Case Manager provides mental health and substance abuse services to individuals, coordinates care with community partners, and links clients to resources. They conduct assessments, develop treatment plans, and maintain documentation. This role also involves providing supportive counseling and fostering relationships with clients while ensuring appropriate boundaries.

Assists with overseeing clerical tasks, managing office supplies and equipment, developing processes, and ensuring efficient office operations. Responsible for staff training and supervision. Requires effective communication, computer literacy, organizational, and scheduling skills.

Provides emotional and informational support for program participants and their families, assists with access to positive social connections and recovery support services, ensures continuity of care, and shares lived experiences to provide education, support, and hope. Collaborates with team members and community partners. Reports to work daily, develops therapeutic relationships, assists in goal setting, provides individual and group peer support services, and advocates for clients. Educates the community about peer support services and completes all required agency trainings.
