About C-Change

The C-Change Program supports the transformation of the lives of youth who are at risk or have been identified as sexually exploited minors by focusing on supporting stabilization, reducing mental health symptoms, self efficacy and self determination, community building, and connecting youth with other resources. Our specialized approach provides mental health screening, advocacy, clinical case management, and psychotherapy to at-risk youth. C-Change provides services to youth who are up to 21 years old and reside throughout the Bay Area.

What is the role?

The Service Coordination Specialist will collaborate with C-Change directors and external agencies like CDSS and SFSOL to coordinate services for exploited youth. This role includes supporting other C-Change staff with onboarding, orientation, consultation, cross-program collaboration, and other assigned duties. The position requires strong advocacy, administrative, and facilitation skills, along with creativity, self-motivation, flexibility, and adaptability. As the likely first point of contact for referral parties, the Specialist will assist with the intake process and conduct needs assessments to facilitate service engagement and stabilization.

The Service Coordination Specialist is responsible for overseeing and actively participating in the care and treatment of children and families, ensuring alignment with the Child and Family Team (CFT) Plan for lasting improvements. Using a trauma-informed, strengths-based approach, the Specialist advocates for youth, ensuring their voice and choices are central to their care, fostering empowerment and effective steps toward healing.

The Service Coordination Specialist reports to both the Program Director and Assistant Program Director, providing feedback and support related to program needs, staff morale, and culture building.

Essential Job Duties: 

  • Schedule and Organize Multidisciplinary Meetings: Regularly arrange and facilitate meetings with providers, youth, family, and natural supports to coordinate services effectively.
  • Collaborate with Partners: Work as an equal partner with families, natural supports, and formal system partners to facilitate a strength-based, culturally sensitive, and needs-driven service planning process.
  • Conduct Initial Intakes: Perform initial assessments with youth, referring parties, and other collateral contacts to evaluate needs and support linkage to mental health and other necessary services.
  • Provide Short-Term Support: Offer immediate engagement and support to youth, including case management, harm reduction and safety planning, and psycho-education on trauma impacts, commercial sexual exploitation, and the cycle of change, all within a safe and affirming environment.
  • Orientation and Training: Assist in providing orientation, consultation, and training on community-based services, program policies, systems navigation, and trauma-informed practices, especially for CSEC (Commercially Sexually Exploited Children).
  • Collaborate on Interventions: Work with HSA (Human Services Agency) and SFSOL  providers on screening, capacity building, interventions, and support services for youth impacted by sexual exploitation.
  • Supervision and Training: Participate consistently and punctually in individual and group supervision, program meetings, and training sessions.
  • Work Location and Travel: Work from Alameda County/San Francisco County sites and be willing to travel within a 90-mile radius of the Fruitvale offices.
  • Engage Youth in Treatment Planning: Engage youth in exploring their needs and goals for treatment and care, including navigating EPSDT (Early and Periodic Screening, Diagnostic, and Treatment), SFPSDT (San Francisco Periodic Screening, Diagnostic, and Treatment), SFSOL, and other funding resources.
  • Support Program Goals: Participate in special projects and tasks as directed by the Program Directors to support the advancement of C-Change program goals.
  • Other duties as assigned.

Position Qualifications:

  • Bachelor's degree in psychology, criminal justice, or a related social services field preferred.
  • Minimum 3 years of experience in targeted case management and rehabilitation services for Transition-Aged Youth (TAY).
  • Experience with high-risk youth, including those exploited (CSEC) or with severe mental health issues (SPMI).
  • Knowledge of foster youth needs, strengths, and relevant administrative rules in Alameda and San Francisco Counties.
  • Proven ability to engage clients positively, solve problems, and collaborate effectively in diverse, multi-disciplinary teams.
  • Familiarity with community resources, trauma-informed care, and culturally relevant services in Alameda and San Francisco Counties.
  • Experience with EPSDT Medi-Cal billing, quality assurance, and team-based care principles.
  • Strong computer skills and knowledge of Diversity, Equity, and Inclusion (DEI) practices.
  • Ability to work independently and within multidisciplinary teams across various settings.
  • Clean California DMV record; travel is required.

Position details and compensation:

This is a regular, full-time position. Starting salary is $66,000 per year, depending on experience, and includes benefits package, 403b retirement contribution and performance-based compensation plans. Added 10% Spanish language differential, if applicable.
